Realizing the health disparities that exist for low income individuals in our community, the Urban League of Greater Chattanooga launched the Community Health Navigator program to close the health care disparity gaps among the uninsured. Racial health disparities are widespread on a local and national level with African Americans in Hamilton County twice as likely as Caucasians to be diabetic and twice as likely to be uninsured, according to a study by the Ochs Center for Metropolitan Studies. Providing several outreach services, the program focuses on helping individuals navigate the health care resources available in our community to reduce these negative health outcomes. Services include: monthly free health screenings in the community, mammogram screenings, referrals for primary care, free prescriptions, free exercise programs, free nutritional counseling and free eyeglasses. To qualify for participation, an individual must be uninsured and meet other eligibility criteria.

Impact:

  • The program has served 264 individuals since it was launched in March 2009
  • The program is saving lives. For example, one uninsured female participant received a free mammogram through the program and was diagnosed with Stage 3 breast cancer; she is now undergoing treatment. Without the program, she did not know where to go for medical assistance.
